Failure to submit required paperwork will automatically cancel your appointment.
Long Distance Consultations: If you would like to schedule a long distance consultation, please send a separate email request in addition to making your appointment.
Cancellation Policy: If you need to cancel or reschedule, please do so at least 24-48 hours in advance. Late cancelation and show appointments will be charged a $50 fee.
